Maybe you have been nominated by someone else who noticed you do something to help our Sun Devil community or you want to take a moment to recognize the efforts of someone else. Either way, we are happy you are here and contributing to our thriving community!

What actions are you taking to contribute to a thriving community? Maybe you have done something recently, could be something big or small, to help out a fellow Sun Devil in need. If so, tell us about it!

As a reminder, this form is not confidential and will be reviewed by professional staff at ASU who are mandatory reporters. Thus, if there is concern for another’s safety or if there are reported violations of the Student Code of Conduct, an ASU staff member may be following up with you to offer support and further assistance. Situations in which we are working to support our fellow community members can be difficult to navigate and you deserve support.

Remember, there are a variety of different resources available to students who may need support. While a few are listed below, in the event of an emergency you should dial 9-1-1. 24/7 Crisis support is available through ASU’s dedicated EMPACT hotline at 480-921-1006.

What have you done to contribute to our thriving community at ASU?

You can pick-up your T-Shirt or giveaway item Monday through Friday, 9am to 5pm in the Sexual and Relationship Violence Prevention office in suite 392 of the Student Services building at the ASU Tempe campus.
